3. Letter of November 28, 2011, from Shatltl A. Whitehead, Fire Chief, requesting an extension of
vacation carry over for James Montgomery, Fire Captain; John Omech, Fire Lieutenant; Kevin
Pilate, Fire Lieutenant; and John Unsworth, Fire Lieutenant, due to extenuating circumstances.
4. Letter of November 21, 2011, from Nicole Hunter, Clerk I, noted by Kathleen Monroe, City
Librarian, requesting an extension of an unpaid medical leave of absence.
5. Departmental Correspondence of November 22, 2011, from Timothy Holt, Sr. Fire Captain, as
approved for submission by Shatltl A. Whitehead, Fire Chief, requesting an extension of the three
year retirement limit for excess vacation accrual.
